Please submit this application if you will study abroad on a provider program (e.g. CIEE, IES, TEAN, SRAS summer program, or another provider not listed here).

Please do not submit this form if you will study on a Stetson faculty-led or exchange study abroad program.

**Note** This application must be submitted before WORLD can fill out a Home School Nomination/Advisor Statement for your study abroad provider.
